This is a form stating whether you agree or not to share your detailed medical record with other health professionals who may need to provide treatment for you. More information is available here Information Leaflet on sharing your record

**  Summary Care Record for Emergency Care

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. If you wish it can also contain additional information about your health.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.  For more information on the NHS Summary Care Records, see the attached information sheet Summary Care Record Information Sheet or go to the website at:

Changes to Personal Details

If you change your phone number, address or name please let us know as soon as possible, so that we can update your records. This is important, especially if we need to contact you.
