New Patient Registration

Welcome to Abbey Medical Centre, this practice accepts patients who have moved into, or are living in, our catchment area. If you meet these criteria and would like to register with us, then you will need to fill in the following paperwork (links below). Once this paperwork is complete you will need to bring this paperwork to reception in person. Please do not post these forms to the practice; you will need to come into the practice to have your identification verified.

Please ensure you bring both the following when you do so:

· one photo ID (e.g. driving license or passport)

· one proof of address (e.g. utility bill)

Ensure you read the documents carefully and fill in all pages to ensure your application is completed fully, failure to do so will lead to us having to contact you to come back and complete the missing information.

Summary Care Record for Emergency Care

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. If you wish it can also contain additional information about your health.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

Please fill out a Summary Care Record Form and return to the surgery stating whether or not you wish to share your emergency care record.  For more information on the NHS Summary Care Records, see the attached information sheet Summary Care Record Information Sheet or go to the website at:

Consent to Share Information to other Health Professionals.

You are also required to complete a form stating whether you agree or not to share your detailed medical record with other health professionals who may need to provide treatment for you. Changes to Personal Details

If you change your phone number, address or name please let us know as soon as possible, so that we can update your records. This is important, especially if we need to contact you.
