How to Register

To register with our practice we need a number of registration forms to be completed. These can be collected from the surgery, or downloaded here to be printed and completed.

Patients who are homeless, new to the country, or without proof of address of ID are able to register with us as patients. 

*  Consent to Share Information to other Health Professionals.

This is a form stating whether you agree or not to share your detailed medical record with other health professionals who may need to provide treatment for you. More information is available here Information Leaflet on sharing your record

**  Summary Care Record for Emergency Care

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. If you wish it can also contain additional information about your health.

Changes to Personal Details

If you change your phone number, address or name please let us know as soon as possible, so that we can update your records. This is important, especially if we need to contact you.
